My question is simple, in an actual sexual encounter how should one use box breathing? It’s a pretty obvious thing and if you are doing foreplay you can’t really subtly do box breathing

I think you do it just before - puts you in a calm/neutral headspace

Doing box breath before sex is a great idea!
The deeper exercises on Mojo can help you understand the root of what triggers you to go into fight or flight, retrain your attention, and show your subconscious there’s nothing to worry about (as well as increase pleasure and connection ofc). You won’t be relying on box breath alone to stay in the zone, so don’t worry if you notice box breath has its limits
